$45-$52 per hour
Responsibilities:
The Dementia Care Program Manager is a multifaceted position that requires intimate knowledge and skills in working with individuals at risk for or who have been diagnosed with Alzheimer's Disease and Related Disorders (ADRD), and those with Intellectual and Developmental Disabilities.
The DCS also coordinates educational trainings to increase the dementia capability of Snowline and throughout our community.
The Dementia Care Specialist uses a systematic approach that is influenced by evidenced based practice, behavioral symptom management education and interventions to aide in caregiver stress relief.
The DCS also develops and maintains professional relationships with community physicians, discharge planners, social workers, community service agencies, Veterans' Affairs, and other related healthcare providers.
Your Experience and Education:
Masters in Gerontology, Public Health, Social Work, BSN, or equivalent degree in area of Brain Science or Aging Must possess a certification through the National Council of Certified Dementia Practitioners or be willing to obtain a certification within 90 days of hire.
A demonstrated ability in casework and counseling, including one (1) year of clinical work experience, preferably in either a healthcare setting or palliative care program Excellent communication and counseling skills, demonstrated skills in assisting individuals and families in problem solving and utilizing community resources Demonstrated ability to efficiently operate a computer, prior electronic medical records (EMR) experience preferred Valid CA driver's license required.
A reliable vehicle that is insured in accordance with state requirements Demonstrated ability to efficiently use a computer required Prior electronic medical record (EMR) experience preferred Occasional evening and weekend availability may be necessary upon occasion Commitment to providing compassionate and patient-centered care Knowledge of trauma-informed care principles preferred Application Instructions:
